Affordable Care Act (ACA) Enrollment
As the preferred partner for strategic and community-based planning in Miami-Dade and Monroe counties, the Health Council of South Florida is committed to assisting with efficient and effective implementation of the Affordable Care Act in our community.
MD-HAN Community Health Worker/Patient Navigator Work Group
The Community Health Worker/Patient Navigator (CHW/PN) Work Group of the MD-HAN is focused on bringing awareness to the work of CHWs in Miami-Dade County. Their primary goal is to develop a platform for the exchange of best practices and lessons learned, which are then disseminated throughout the Network in order to contribute to the enhancement of service delivery among CHWs and PNs.
Miami Dade Health Action Network (MD-HAN)
The Miami-Dade Health Action Network (MD-HAN) is a multi-sector cross collaborative, which includes over 2,300 interested Community Partners, serving as the neutral convening platform to address issues regarding the promotion of an integrated safety-net system and access to health care in Miami-Dade County. The mission of the MD-HAN is to serve as the catalyst and resource for safety-net innovation, collaboration and policy direction to promote accessible, coordinated and affordable care to the uninsured and under-served of Miami-Dade.
Partnerships to Improve Community Health (PICH)
Supports implementation of evidence-based strategies to improve the health of communities and reduce the prevalence of chronic disease. PICH builds on a body of knowledge developed through previously funded Centers for Disease Control and Prevention (CDC) programs and encourages collaborations with a multi-sectoral coalition to implement sustainable changes in communities where people live, learn, work, and play.
SE Florida Cancer Control Collaborative
The Health Council of South Florida (HCSF) serves as the lead oversight agency, facilitator and convener of the Southeast Florida Cancer Control Collaborative (SFCCC). The SFCCC is a voluntary organization whose mission is to improve access to cancer care and to decrease the burden of cancer through a united effort by the promotion of education, advocacy, and research for the people of Southeast Florida. The organization is one of five regional cancer collaboratives throughout Florida; with the Southeast Florida region defined as Broward, Indian River, Martin, Miami-Dade, Monroe, Okeechobee, Palm Beach, and St. Lucie counties.
